From now on, the Education Welfare Team within the Local Authority will manage all referrals. These will be considered by our Pupil Placement Medical Needs Panel (PPMP).

The PPMP is a multidisciplinary panel comprising representatives from the Local Authority, headteachers from Pupil Referral Units, social workers, and SEN specialists. The panel meets weekly to review referrals submitted by schools via Microsoft forms and determine the most appropriate outcome for each young person.

Please use the link below to access the new referral form effective immediately:

Pupil Placement and Medical Needs Panel Referral Form

To ensure eligibility,  pupils will need to come under medical needs or Section 19 characteristics.

Please include information in the referral form in addition to where you’re requesting details of school support in place: please attach provision map and two cycles of Assess Plan Do Review (using either SEN support plan or EBSA support plan if CYP is experiencing EBSA).

For guidance, please refer to the following resources:

Following the panel’s decision, you will receive confirmation of the outcome, including whether alternative provision will be offered to the referred young person.
